Anytime a human being gets cut (with surgery or trauma), scars heal the following way: Initially they are red, raised and hard. Within 6-12 months, they are flat, white and soft. Younger people (
<18 years old) have scars that might stay active or red for up to 2 years. Older people (>65 years old) have scars that might stay active or red for less than 3 months.

This is a frequent problem seen after this procedure because plastic surgeons are traditionally taught to place the implant 2 cm below the crease - which is not correct, especially in older patients! Unfortunately this is a very difficult problem to correct. That is why it needs to be done correctly the first time.

During the surgery, the dissection, internal sutures, and most of the work is done differently on both sides to try to make people as symmetric as possible. The truth is as soon as you stand up, things can fall differently, and things can scar down differently. No matter what you look like a few weeks after surgery, at the 3 month mark we have a good idea where you’ll end up but realistically by the six month mark what you have is usually what you will have. Sometimes it can take up to a year at certain patients, especially revisions. At that time, revisions to further enhance asymmetry can be discussed. Patience is required during the initial postoperative period. I always tell my patients that 100% of people are asymmetric before surgery and they can expect to still be asymmetric after surgery.

82 year old 9 years after breast reconstruction with saline implants after breast cancer who noticed decreasing breast implant volume over the previous four weeks. Patient has the following options at this point - exchange as an outpatient procedure, removal of the implant as an in-office procedure under local anesthesia, or continuing to watch the deflated breast implant if they refuse any surgical intervention (I do not recommend this, but some older patients opt for this choice as did this patient). I kept following up with her in clinic every 6 months until she passed away from other causes 8 years after this picture.

She had a lot of rippling and wrinkling of the implant especially when she leaned over. She complained that they did not feel natural and were very palpable. These are the complaints I usually hear from women who have implants under the breast and not under the muscle. She also wanted a slight reduction in size. Only her old periareolar scar was used.
