Patients look very different when they are looking down, animating or moving around. In this picture, you will notice the patient is looking downwards and trying to touch her chin to her chest. This actually reveals a much more dramatic result from her mini neck lift then you would expect to see, especially because you're not really cutting out much skin with such a small scar.

Patient had enough skin laxity to warrant a full scar neck lift but did not want the anterior scar. So the scar she got with this new minimally invasive surgery was a 3 cm lateral neck scar tucked under her jaw line and a 1 cm scar in the hair.

This is not a thread lift which does not give much improvement and does not last. After 1 year most thread lift patients notice only a 1-2 mm difference. Here, the SMAS muscles are tightened with 2-0 PDS internally which absorbs after four months and these are the same sutures we use for facelifts and full neck lifts. The result is much more dramatic.

A 65 year old male patient before and 6 months after a direct necklift done in the office under local anesthesia which took only 20 minutes. Even though he was a candidate to get other procedures such as a facelift, cosmetic eyelid surgery, as well as injectables, the only thing that bothered him with his neck and he was willing to tolerate a small T scar. This is a very gratifying operation especially for men who can hide the scar in their beard. His retruded chin was also elevated (pushed upwards) with the procedure to look more normal and even his jowls were improved.

The hardest part to this operation is the initial postoperative period and looking at the red thick scar but as with all scars, it usually softens within 6-12 months to become a soft white line in most patients.

Patient also did not mind having an additional two scars - one in a lateral neck ring and one behind the ear in her hairline. These two additional small 1 centimeter incisions allowed a more powerful lifting procedure to further improve her neck. Her neck had a lot of skin laxity and she really needed a full neck lift scar to properly address this. We call this a modified mini lateral neck lift and this procedure has not been described by anyone previously.

The Chicago Neck Lift Plastic Surgery Scars:

Anytime a human being gets cut (with surgery or trauma), scars heal the following way: Initially they are red, raised and hard. Within 6-12 months, they are flat, white and soft. Younger people (<18 years old) have scars that might stay active or red for up to 2 years. Older people (>65 years old) have scars that might stay active or red for less than 3 months

48-year-old female 1 day after only a lateral mini neck lift. This procedure only has two small 1 cm scars, one on the lateral neck, one behind the ear. Notice how the puckers are most prominent right after surgery especially because no skin is removed. The inside PDS stitch lifts the Smas/Platysma muscle and tightens the sling while absorbing over 4 months. This is not a thread lift which does not show much permanent difference after the first week. These puckers should be massaged and usually flatten very quickly.




There are four different options to improve a neck. Depending on how much extra skin you have, fat, and how lax your muscles are these options will be discussed during your private consultation. First option is just liposuction. Second option is a mini neck lift through a small scar in the submental crease. This pulls the neck medially or inward. Next option is a mini lateral neck lift, which has two small scars, and pulls the neck laterally or out to the side. Finally, the last two options are a T or Z neck lift, where the submental skin excess is removed and a full neck lift, which includes the T or Z scar as well as a vertical scar all the way down to the sternal notch. This last option is the most aggressive, but has the most scarring.

In a lower blepharoplasty, typically a canthopexy is done at the same time to tighten the loose orbicularis muscle and give a more natural look while diminishing ectropion or lateral scleral show.

A Midface or cheek lift is done through the same incision as a lower blepharoplasty with the addition of a small incision in the hairline. The muscle is lifted in two directions, one with the lower blepharoplasty and canthopexy, and secondly, in a different vector superiorly to lift the mid face or cheek. Most people lift this laterally, but I have changed my approach, because that did not look as natural and it was not as powerful a lift. The midface or cheek ages and descends downward. Please remember that with a mid face or cheek lift, you are also getting a lower blepharoplasty at the same time. It is included and not a separate procedure.

Most surgeons will lift the muscle or SMAS in 1 direction. I have evolved my technique over the years to lift it in 3 separate directions, which results in much more powerful lift, as well as a more natural look. The 3 vectors are determined the day of surgery, depending on what priorities the patient has in terms of jowls, jawline, neck etc. Even though this improves the midface about 30% and the neck, further work can be done with a midface or cheek lift, as well as additional work to the neck, depending on the patient's examination and their expectations.

With a full scar neck lift, the vertical scar can be seen but the submental scars typically are not exposed unless a patient is looking backward and fully extending the neck.

There's not much we would recommend for laser treatments here, but we would normally recommend everywhere else. There's a little overlap with laser hair removal and we do not want to promote and remove any hair because the beard hair camouflages the scar. So within a man's beard is the one area we don't typically recommend postoperative laser scar treatments.

62 year old male 6 months after a full scar neck lift. He was very comfortable with the scar compared to what he had before surgery. Obviously most men can camouflage most of the scar in their beard line. The scar can be tailored and be longer or shorter depending on what excess skin bothers the patient. It can even be fully within the beard line. Once patient's scars are mature and they are happy with their size and orientation, they can get FUE (minimally invasive hair transplants without cutting and big occipital scars) into the mature white surgical scars to further camouflage them. For areas outside the beard line, they can also consider getting cosmetic tattooing to make the white scar more flesh colored and thus less noticeable.
